Official explanation
For lost/stolen cards only: Bring your application to a Gardaí station. The Gardaí must complete, sign and stamp the Gardaí Report on Section 1. If your digital tachograph card is lost, stolen, damaged, or malfunctioning, you should apply to the Road Safety Authority to have it replaced as soon as possible. If your digital tachograph card is damaged or malfunctioning, it must be returned to the Road Safety Authority with your application for a replacement.
